Due to the current situation with Coronavirus, the service for Online Appointment Bookings has been withdrawn. All other issues with Online Services are ongoing and still being investigated.
This practice provides online services for appointments, prescriptions and access to medical records. Registration for appointments and medical records is required to obtain the practice ID, Access ID numbers and password which you will require. Please complete and print the registration form and visit the surgery to register for online services, bringing two forms of ID with you, options shown below (one of which MUST include a photograph). Once your registration has been approved by the practice you will be issued with a 'Registration Token'. Use the information on the registration token to complete the registration process online; you can do this by clicking the link www.patient-services.co.uk
Documents accepted: either Passport or Driving licence (photographic) AND one other of:
- DBS enhanced disclosure certificate, Mobile telephone contract, Buildings/Contents Insurance certificate, Vehicle Insurance certificate,
- Bank/Building Society/Credit Union current account, Credit card account, Council Tax bill.
Regrettably this service is not available for under 16 year olds.
